What You’ll Do:

• Act as an advocate and emotional support system for residents and their families.

• Provide guidance on resident rights, care planning, and transitions of care.

• Coordinate services and resources to enhance resident wellbeing — from counseling to arranging personal items or support services.

• Serve as a key member of our interdisciplinary team, working closely with staff and family to address individual needs.

• Support residents through grief, adjustment, and life changes with compassion and care.

• Document social services needs and ensure individualized care plans reflect evolving goals.

What We’re Looking For:

• A licensed Social Worker in good standing, or a degree in social work or related human services field (sociology, psychology, gerontology, etc.).

• At least one year of supervised social work experience in a healthcare setting.

• Someone who is patient, empathetic, a great communicator, and thrives in a fast-paced, resident-first environment.
